As a medical social worker, I use advocacy every day to ensure all patients are treated fairly and have a safe discharge plan in place.

As a medical social worker, I use critical thinking every day by evaluating each clinical case, interviewing patients and families, making observations, and making informed decisions.

As a medical social worker, I use boundary setting every day by professionally working with patients and families, abiding by confidentiality rules, and avoiding taking work home mentally and physically.

Throughout this course, I have shown great improvements in communication. When I first began this internship, I was unable to communicate with patients effectively due to fears, being uncomfortable, and having no experience.Now, as we are nearing the end of the course, I can communicate effectively with all patients regardless of cultural background, medical diagnoses, and mental status. I am confident when entering patients' rooms alone and present myself professionally

Throughout my time at the hospital, I have shown incredible improvements in my ability to lead patients in discharge planning. At the beginning of my time at the hospital, I was uneducated on medical discharge planning and how to get patients from the hospital to home or rehab safely.Now, as we are nearing the end of the course, I can diligently work with patients and families on safe discharge planning to home or rehab. I am knowledgeable about each of the rehab facilities the hospital works with. I can make educated assumptions regarding the discharge and speak with patients independently about their needs going forward.

Throughout my time at the hospital, I have shown incredible improvements in communicating with rehabilitation facilities and medical transport companies. At the beginning of my practicum, I was hesitant to speak with any facility or company regarding patients, as I was nervous about what to say and felt I was not ready or able to communicate effectively.Now, as I am nearing the end of my practicum, I can call all facilities and companies regarding patients' needs and transportation requests. I can call liaisons at rehabilitation centers and communicate with them about patients' needs, ask if they can take the patient once discharged, and advocate for my patient's wishes and needs as directed by medical personnel.

Throughout this course, I have shown great improvements in speaking with patients about their financial situations. At the beginning of this experience, I was uncomfortable speaking with patients about their financials and was unsure of how to approach this topic. I struggled with how to respond to patients who stated they were poor, living in poverty, or who were homeless.Now, as we are nearing the end of the course, I can confidently speak with all patients about their financial situations. I am knowledgeable about community resources in Charlottesville and surrounding areas. I can advocate for my clients by providing patients with Medicaid applications and ensuring any homeless patients are connecting with Region 10, the Department of Social Services, and local shelters.

Throughout this course, I have shown great improvements in insurance policies. At the beginning of this practicum, I did not know about Medicare, Medicaid, and their supplements. Nor did I know much about any other insurance company and how they work. I could not advocate for my clients effectively because I did not know what their insurance would cover.Now, I am educated on insurance coverage and can guide my patients in getting their needs met. I know about various insurance companies and can call regarding patients who are unable, but need more information.
