Action forms communicate all post-assessment activities to the VA. These forms are vital for showing we are connecting the client to services and the client's overall standing in the program. At minimum, participants should have Action Forms completed at program enrollment and exit. They should also be completed every time there is a Fox Grant related referral for the participant

What are Action Forms?

Complete screening assessments. If they score "No risk" on the Columbia, they are ineligible. Submit an Action Form stating they are ineligible, any referrals you made, and you are done! If they score "Low/Moderate" they are eligible for services. Submit an Action Form for any referrals you made and proceed to Enrollment Assessments. If they score "High", you must submit an Action Form for what referrals you made and go to Enrollment Assessments.
