CASE SCENARIO

Stephany Jones, a 28 years old G2P1 went to Brock Maternity for her initial prenatal check-up. Stephany asked you about different signs and symptoms that she experienced during pregnancy. She also wants to learn the different diagnostic procedures that a pregnant woman usually undergoes.

DISCUSSION

Determine the following signs and symptoms whether it is presumptive, probable, or positive sign of pregnancy.
2.A

Stephanie stated “My doctor asked me to have an ultrasonography will you give me an overview about this procedure?” Answer the following questions:

A. What are the types of ultrasound usually requested during pregnancy?

1. Transvaginal UltrasoundDuring this type of ultrasound, a wand-shaped transducer is placed inside your vagina. Transvaginal ultrasounds are usually used in early pregnancy to verify your pregnancy, check your baby’s heart rate and position, look for signs of ectopic pregnancy, and detect how many fetuses are present. But they may also be used later on in your pregnancy if abdominal ultrasounds aren’t providing enough information.

2. Transabdominal Ultrasound Transabdominal ultrasounds use a transducer that is placed on your abdomen to view your baby and check on your pregnancy. It is routine to receive a thorough transabdominal ultrasound about 18 to 22 weeks into your pregnancy, but they can be used later on in your pregnancy if anything of concern comes up that needs to be investigated or monitored.

3. 3D Ultrasound This type of ultrasound can be done transvaginally or transabdominally. 3D ultrasound is used when more detailed images of your baby are needed. It can help detect conditions such as neural tube defects and facial abnormalities.

4. Doppler Ultrasound Doppler ultrasounds are used to observe movement within blood vessels and can provide information about your baby’s blood flow, as well as the blood flow in your uterus and placenta.

5.Fetal Echocardiography Fetal Echocardiography allows doctors to see a detailed image of your baby’s heart and heart structures. It may be used if a congenital heart defect is suspected

There are 2 types of ultrasounds conducted during pregnancy. First is the Transvaginal ultrasound, with this type of fetal ultrasound, a wandlike device called a transducer is placed in your vagina to out a sound waves and gather the reflections. Transvaginal ultrasound also might be done if a transabdominal ultrasound didn’t provide enough information. Second is the Transabdominal ultrasound, a transabdominal fetal ultrasound is done by moving a transducer over your abdomen.

Differentiate the 2 types of ultrasound conducted during pregnancy.

2.B
  • Movements of the fetus
  • Size of fetal head, abdomen, thigh bone and the estimated fetal weight
  • Position of the placenta and appearance
  • Measure of the amount of amniotic fluid
  • Assess the blood flow of the placenta, fetus and baby’s brain using doppler ultrasound

What are the important information about fetal well being that the healthcare provider may get from the ultrasound report?

2.C
  • Explain to patient the procedure and its purpose.
  • Help the patient with the positioning.
  • Ask the patient to relax while the procedure is going on.

What are the important nursing management in assisting the client during ultrasound.
