(+) S + VERB ... Every Sunday, you play basket. He/She/It -> On Mondays, he starts a new book. (-) S + DON'T / DOESN'T + VERB ... Every Sunday, you don't play basket. He/She/It -> On Mondays, he doesn't start a new book. (?) DO / DOES + S + VERB ... Do you play basket every Sunday? He/She/It -> Does he start a new book on Mondays?

PRESENT SIMPLE

don't forget to add -s when you are talking in 3rd person singular (he/she/it)

OTHER VERBS

Since you are using the auxiliar DID in the negative and interrogative structures, you will always use the infinitive after it!

I/He/She/It -> Last Monday, he wasn't at the school. (?) WAS / WERE + S ... You/We/They -> Were we with Mery yesterday? I/He/She/It -> Was he at the school last Monday?

(+) S + [ VERB(-)ED / IRREGULAR VERB ] ... Yesterday, you played paddle tennis. Last Wednesday, they won the contest. (-) S + DIDN'T + VERB ... Yesterday, you didn't play paddle tennis. Last Wednesday, they didn't win the contest. (?) DID + S + VERB ... Did you play paddle tennis yesterday? Did they win the contest last Wednesday?
